Be Prepared

Emergencies are not planned.  But there are some things you can do now to prepare for such an event.

Insurance
Keeping the most up-to-date insurance information with you at all times will enable emergency caregivers to quickly process your paperwork and proceed with your care.

Record of Current Medications
Please bring a record of current medications.  If you do not have one, simply bring your medication bottles with you.

Permission to Treat Your Child at Mercy
Nothing should stand in the way of emergency care for your child. Should your son or daughter require emergent care in your absence, it is important that we have his/her most current medical history, as well as your consent for treatment. Click below to access a consent form.

Fill the card out completely and either drop it in the mail or fax it to the Mercy Emergency Department (address and fax number provided on form).  We will keep this on file for one year. You will be contacted by hospital staff for verification of the consent form upon receipt.
