The Congestive Heart Failure Disease Management Program
coordinates care for congestive heart failure patients.
With this program, an advanced nurse practitioner and
a registered nurse work with individuals and their families.
Their joint efforts with your physician can help produce
the best results in managing healthcare issues.
Trust and respect are developed between the participant
and the nurses to set mutual healthcare goals that meet
individualized needs. The program offers:
- Hospital or clinic visits and telephone contact
from a registered nurse to help with your healthcare
needs.
- Assistance by a nurse with communication and coordination
among healthcare team members during your hospitalization
and after discharge.
- Monitoring of your health status by a nurse.
- Assistance with management of medications.
- Communication with your physician to help coordinate
your healthcare.
- Help with learning how to take care of yourself
when you leave the hospital.
- Assistance with health resources and referrals.
- Telephone follow-up.
