Interdisciplinary Collaboration

Interdisciplinary CollaborationInterdisciplinary Rounds incorporate a broad membership including nurses from across care settings, social workers, therapists, hospitalists, chaplains, case managers and other care team members, who meet daily to discuss the patient’s progress, and communicate and collaborate on next steps. The agenda cultivates active involvement by each participant, and uses data and evidence-based tools in collaborating on the most effective way to deliver care. We believe the best patient care requires excellent teamwork.

Mercy nurses passionately collaborate with other members of the health care team on subjects like:

  • Clinical safety
  • Informatics
  • Care planning
  • Employee recognition
  • Clinical policies and procedures
  • Quality indicators

Nurses have dynamic relationships with other members of the interdisciplinary team, on intra-departmental, inter-departmental and inter-organizational levels. Increasingly, nurses work together with other clinicians across the continuum to ensure effective transitions in care and the best outcomes. Our goal is for the patient to achieve the highest level of success possible at Mercy and at subsequent levels of care.
