Nothing should stand in the way of emergency care for
your child. Should your child require emergent care
in your absence, it is important that we have his/her
most current medical history, as well as your consent
for treatment. Click on a link below to access a consent
form. Fill the card out completely and either drop it
in the mail or bring it to the Emergency Department
desk. We will keep this on file for one year. You will
be contacted by hospital staff for verification of the
consent form upon receipt.
