Skilled nursing is a transitional level of care that provides a link between acute care and home or nursing home care. If you or someone you love is in this transitional time, skilled nursing can help to provide a continuity of care. In skilled nursing, patients are assisted with independence in their daily care and activities in order to help them achieve their highest level of functioning.
