If you or someone you know has a heart failure diagnosis, the Tele-Minder Program can give you the ability to take more control over your care. It is a specially designed telephone support system for patients with a heart failure diagnosis.

With the Tele-Minder Program, information is collected over the telephone and provided to your physician to follow the progress of your care.
How it Works:

• Each day between 4:00 a.m. and noon you call the Tele-Minder system to report your morning weight and answer simple yes or no questions regarding how you are feeling that day.
• The answers will be provided to your healthcare provider,
• Your healthcare provider will use the information to manage your heart care.

This program allows you to begin taking more control over your care with the confidence that your physician will be kept closely informed of your daily progress, response to medication and dietary therapy.
